The following pages are provided to help prepare you for a great experience at our office. Give us a call if you have any questions. Our dental receptionists and office manager will help you with scheduling appointments, completing necessary paperwork, and billing.

You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

Note to Patient: Upon filling out your first name, last name, email and at least one phone number field (a valid home or cell phone) a button will appear in the upper right corner titled “Save and Exit” . This button allows you to save and exit your form to complete at a later date. The system will send a registration email to the email provided on the form, so that you may setup a login to go back and finalize your registration form online.

Prior to your surgery, you’ll have a pre-op consultation in which we explain exactly what will happen during your procedure. We will also give you pre-operative instructions at this point so that you can prepare for your upcoming appointment. Some of the most common pre-op instructions involve fasting before the procedure, avoiding smoking, and staying away from certain medications. Failure to follow these rules could delay your procedure, and you may need to reschedule entirely.

After your oral surgery, you’ll be sent home with a detailed list of instructions on how to properly care for your teeth and gums. It is common to experience some discomfort, bleeding, and swelling, and we will provide you with prescription medication and home care advice on how to best deal with these issues. It is important to follow this guidance closely, as compliance can help you to avoid unnecessary pain and potential complications.

Remember that if you’ll be under anesthesia that you will need someone to drive you home, and this person should plan to sit and wait during your procedure. Your escort can also be useful in helping you to understand and remember your post-operative instructions.

Our receptionists and office manager will help you schedule appointments and complete necessary paperwork. They will also answer any billing questions that you may have.

Patient Appointments

Patients are seen by appointment only. We make every effort to be on time for our patients, and we ask the same courtesy from your family. If you cannot keep an appointment, please notify us immediately, giving a minimum of 48 hours notice to avoid any missed appointment fees. We realize that unexpected illnesses and emergencies may occur, but we ask for your assistance in this regard.

Our office strives to schedule appointments at your convenience and when time is available.

Insurance & Patient Co-payments

In order to best assist you, it is important that you submit your dental and/or medical insurance information to us with your New Patient Paperwork a minimum of two business days prior to your scheduled appointment. In addition, you will want to bring a dental benefit book that your carrier should be able to provide. Our business team will be happy to help you understand your policy.

Should you be diagnosed with treatment services, we will be able to provide you with a detailed plan which will give you an estimate of how much your insurance company may pay for each procedure, as well as the portion you will be expected to pay at the time of service.

Professional care is provided to you, our patient family, and not to an insurance company. The insurance company is responsible only to the patient as a benefit from the individual or employer plan purchase, and the patient is responsible to the doctor.

Financial Policy

In an effort to reduce the cost to you, the patient, while maintaining the highest level of professional care, we have established the financial policy as follows:

Payment of any account balance is due in full within 20 days of statement date
We gladly accept Visa, Discover, American Express or MasterCard payments
Payment plans are available through Care Credit
